29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly, and maintained in a safe condition.    Drill site: On or about April 05, 2017, at the mud tank, employees were exposed to uneven walking-working surface due to a metal sensor floor cap being lifted approximately 3-inches above the surface. This condition exposed employees to trip hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts:    Drill site: On or about April 05, 2017, the draw-works on the rig floor was not provided with a guard to protect employees from hazards such as those created by point of operation, ingoing nip points, rotating parts. This condition exposed employees to caught-in hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to the joints or terminal screws:    Drill site: On or about April 05, 2017, the power cord to the air compressor 46 located in generator power house did not have an effective strain relief device.  This condition exposed employees to electrical shock hazards.    Orion Drilling Company, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 961038, citation number 1, item number 3 and was affirmed as a final order on 04/21/2014, with respect to a workplace located at S. of CR 405 on FM 1680, Markers 1-H Well, Flatonia, TX 78941.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:     Drill site: On or about April 05, 2017, the air conditioning unit condenser electrical control box  outside the tool pusher's trailer had an unused opening. This condition exposed employees to electrical shock hazards.
