29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location: 20 Arrowhead Farm Road, Boxford MA 01921  On or about 8/19/2013 employees were not protected from falling while working from a height greater than six feet below a lower level.  Brian Duggan dba Danvers Roofing, was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13) was contained in OSHA Inspection 316284140, Citation Number 01, Item Number 1, Issued 02/15/2012  with respect to workplace located at 45 Locust Street, Danvers, MA, 01923.  The final order date of the citation was 10/6/12
