29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501   (a) Location - 14 Bow Street, Danvers, MA:  On or about 7/15/14, the employer failed to provide fall protection for an employee working on a roof fourteen (14) feet above grade.   The Brian Duggan dba Danvers Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspections number 931707, citation number 1, item number 1 and was affirmed as a final order on 12/16/2013, with respect to a workplace located at 20 Arrowhead Farm Road, Boxford, MA.  The  Brian Duggan dba Danvers Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 316284140, item 1, citation number 1, item number1 and was affirmed as a final order on 10/6/2012 , with respect to a workplace located at 45 Locust Street, Danvers, MA.   The Brian Duggan dba Danvers Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 315655613, citation 1, item 3 and was affirmed as a final order on 8/16/2011, with respect to a workplace located at 107 Locust Street, Danvers, MA.
