29 CFR 1926.50(c): In the absence of an infirmary, clinic, hospital, or physician, that was reasonably accessible in terms of time and distance to the worksite, which was available for the treatment of injured employees, a person who has a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training that can be verified by documentary evidence, was not available at the worksite to render first aid:    At the project on or about August 20, 2013, and at times prior thereto, employees were not trained in first aid. Medical treatment facilities were more than four minutes from the project. The employees said they were not trained. The employer never provided any evidence of training or information when requested.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws:    Near the middle of the project on or about August 20, 2013, the outside insulation was pulled back from the plug on an extension cord that supplied power to a 240 VAC Reyang right angle grinder. The grinder was used to cut openings for windows and doors in shipping containers. Power was provided by a 5kW portable generator.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders:    At the project on or about August 20, 2013, employees stated they were not trained on the use of ladders and did not know that the top and top step of a stepladder cannot be used. Employees used a 6 foot long Werner stepladder to access areas where they cut holes into inter-modal shipping containers for windows and doors.
