Such facilities were not in near proximity to the worksite and was not equipped as to enable employees to remove such substances:   Niking Corporation - Andersen AFB Clear Water Rinse Facility site - On or about August 16, 2013 and at times prior thereto, wash facilities for employees engaged in mixing and placing concrete in forms were not available.  Non-potable water was located where they were mixing the concrete adjacent to the trench but was not adequate for washing.  Green microbial growth was present in the tank.  No soap was available at the concrete mixing and application site.
