29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     a) On or about 8/23/13 in the Forge area; written procedures titled Setting Up Hammers, did not include instructions for identifying the energy sources, shutting down and applying lockout/tagout devices to the energy isolation devices and testing prior to beginning service and maintenance work such as but not limited to die changing.        b) On or about 8/23/13 in the CNC area; employees are involved in setting up the tracer lathe, with two energy sources ( air and electric ), without the use of a written procedure for locking out this equipment. 29 CFR 1910.218(f)(1)(i): The power to mechanical forging press(es) was not locked out when the die(s) were being changed on the press(es):     a) On or about 8/23/13 in the Forge area; the local control circuits are shut off and the air is turn off when dies are being changed on the on the 2500# board forging presses without locks being applied to these energy isolation devices.          b) On or about 8/19/13 in the Forge area; the electrical disconnect in the breaker (motor control) room is shut off and the door to this room locked when the dies are being changed on the 2,000# board forging hammer.       No Abatement Certification Required

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:   a) On or about 8/19/13 in the Forge area; during servicing of the Forge presses, a lock is attached to a latch on the side of the door to the motor control room, which contains the electrical panels for the five lathes and five trim shears in this work area, effectively preventing access to the circuit breakers for the equipment located in the Forge area exposing employees to potential for caught between hazards.     No Abatement Certification
