29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable. a) On or about 10/24/2018, throughout machine shop, employer failed to establish a lockout program that consists of specific energy control procedures, authorized employee training and periodic inspections to ensure employees perform energy control procedures before servicing or doing maintenance such as but not limited to, setting up the Broaching machine, changing blade on bandsaw or changing belts on sanders. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 10/24/2018, in the Steel Department, when employees perform maintenance and servicing activities, such as changing the blade on the bandsaw, energy control procedures were not being utilized, exposing employees to unexpected start of the bandsaw. b) On or about 10/24/2018, in the Deburring Department, when employees perform maintenance and servicing activities, such as changing the belt on the sanders, energy control procedures were not being utilized, exposing employees to unexpected start of the sander. c) On or about 10/24/2018, in the Machine shop, when employees perform maintenance and servicing activities, such as setting up the broachi machine, energy control procedures were not being utilized, exposing employees to unexpected start of the broach machine.
