29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) Shipping Department, Recirculation Line Conveyor, above UPS Line - Maintenance employees climbed onto the recirculation line conveyor in order to clear a jam.  While working from this conveyor to clear the jam, the employees did not wear personal fall arrest equipment to prevent them from falling 20 feet to the floor below, on or about the beginning of August 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    (a) Shipping Department, Recirculation Line Conveyor, above UPS Line - Maintenance employees climbed onto the recirculation line conveyor in order to clear a jam.  While working from this conveyor to clear the jam, the employees turned off the switch for the motor that supplied power to the conveyor but did not apply lockout/tagout, exposing employees to a caught-in hazard or fall hazard if the conveyor became energized, on or about the beginning of August 2013.    (b) C. & J. Clark America, Inc., 240 Kindig Lane, Hanover, Pennsylvania - When doing servicing and maintenance work on machinery and equipment, including, but not limited to, conveyors, maintenance employees turned off the switch for the motor that powered the conveyor but did not apply lockout/tagout to the switch if the employee worked in close proximity to the switch.  Also, in situations where others did not have access to switches, for example, on motor switches elevated above the floor and accessible by scissors lift only, maintenance employees turned off the switch but did not apply lockout/tagout, exposing employees to hazards such as a caught-in hazard if a conveyor or other piece of equipment became energized, on or about August 20, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
