29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 degrees F (37.8 degrees C), were dispensed into containers without the nozzle and container being electrically interconnected. (a) Returned Goods Area - Employees transfer a Category 2 flammable liquid from a 5 gallon container to a 1 quart safety can. The employer did not ensure that the containers were bonded during the transfer, thus exposing employees to burn injuries, on or about March 6, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Maintenance Shop - An employee used an abrasive bench grinder that did not have its tongue guard adjusted to within one-fourth inch of the abrasive wheel, on or about February 24, 2014. No abatement certification or documentation required.
