The gloves had latex rubber palms and cotton mesh backs instead of being made of nitrile or butyl.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more was not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the means described in 29 CFR 1926.403(i)(2)(i)(A)-(D):  At the southeast corner of the project on or about August 21, 2013, and at times prior thereto, energized 240 VAC service entrance cables, about 5 feet from scaffolds and 6 feet form the floor, were not protected against accidental contract. Employees on the floor lifted rebar to employees on the scaffold who inserted the rebar vertically into the concrete masonry units. The insulation appeared intact on the cables, and the splices were wrapped with electrical tape.

All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:  Around the north and west perimeter of the building on or about August 21, 2013, rebars were not guarded. The tops 5/8s inch diameter rebars were about 43 inches from the floor and about 10 inches above the CMUs. There were 26 rebars protruding from the west wall and 32 rebars protruding from the north wall. CMUs were not being laid on these walls.
