29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) On or about August 21, 2013 and at times prior the employees were expose to electrcial hazard in the auto body shop while operating the Hobert Handler 120 welder with damage insulation and pinched outer jacket. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) On or about August 21, 2013 and at times prior the employer did not ensure employees were protected from electrical shock in the auto body shop due to not protecting the wire with shielding to provide strain relief on electrical equipment and electrical cord. 1) Electrical dent puller located in the auto body shop did not have strain relief on the male end of the electrical power cord 2) Electrical cord that was located in the auto body shop, in which the dent puller was plug into did not have strain relief Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
