29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread. Noted on or about 8/21/13 and at times prior thereto: a. Filter Room, ground level - The staiirs leading up to filter area were not equipped with standard height top and mid-rails. b. Husbandry - The stairs leading to the top of the tank were not equipped with mid-rails. c. Octopus Exhibit - The stairs leading to the top of the exhibit were not equipped with top and mid-rails for the full length of the stairs.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: Noted on or about 8/21/13 and at times prior thereto in the following locations: a. Maintenance Shop - The electrical panel box was blocked by boxes and other materials and did not permit a 3-foot clearance. b. Entrance to Filter Room - The main electrical panel box was blocked by yellow floor grating and a railing and did not permit a 3-foot clearance.
