29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.          This violation occurred on or about August 21, 2013, Temtrol, Inc did not develop specific written procedures for equipment such as, but not limited to, the Amada press brakes, Bradbury panel machines and Tridan fin presses when employees are performing servicing and/or maintenance activities such as changing dies exposing employees to the hazards of being caught in the machine.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    The employer does not provide adequate training to employees who perform servicing and maintenance operations such as, but not limited to:      a)         This violation occurred on or about August 21, 2013, in assembly/shipping/receiving area, employees performing setup operations on the Bradbury panel machine were not trained at the authorized level to perform necessary lockout/tagout operations during servicing and maintenance of machinery.        b)        This violation occurred on or about August 21, 2003, in the fabrication area, employee's performing die changing operations on equipment such as, but not limited to, the Accurpress and Amada press brakes were not trained at the authorized level to perform necessary lockout/tagout operations during servicing and maintenance of machinery.

29 CFR 1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:    The employer does not ensure point of operation guarding is place to protect employees from having any part of their body in the danger zone during operation.        a)      This violation was observed on or about August 21, 2013, the Amada press brake #72 point of operation was not guarded exposing employees to the hazards of being caught in the machinery.      b)      This violation was observed on or about August 21, 2013, the Amada press brake #99 point of operation was not guarded exposing employees to the hazards of being caught in the machinery.        c)      This violation was observed on or about August 21, 2013, the Amada press brake #58 point of operation was not guarded exposing employees to the hazards of being caught in the machinery.        d)     This violation was observed on or about August 21, 2013, the Cincinnati press brake point of operation was not guarded exposing employees to the hazards of being caught in the machinery.        e)      This violation was observed on or about August 21, 2013, the Accurpress press brake point of operation was not guarded exposing employees to the hazards of being caught in the machinery.        f)       This violation was observed on or about August 21, 2013, the Mubea ironworker #062 punch point of operation was not guarded exposing employees to the hazards of being caught in the machinery.      g)      This violation was observed on or about August 21, 2013, the Universal ironworker #362 punch point of operation was not guarded exposing employees to the hazards of being caught in the machinery.      h)      This violation was observed on or about August 21, 2013, the Tridan Fin press 1 inch machine point of operation was not guarded exposing employees to the hazards of being caught in the machinery.      i)        This violation was observed on or about August 21, 2013, the Tridan Fin press machine #0301 point of operation was not guarded exposing employees to the hazards of being caught in the machinery.          j)        This violation was observed on or about August 21, 2013, the Tridan Fin press machine #0326 point of operation was not guarded exposing employees to the hazards of being caught in the machinery.      k)      This violation was observed on or about August 21, 2013, the Tridan Fin press machine #0325 point of operation was not guarded exposing employees to the hazards of being caught in the machinery.      l)        This violation was observed on or about August 21, 2013, the Tridan Fin press machine #0009 point of operation was not guarded exposing employees to the hazards of being caught in the machinery.
