29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On the first floor, there were six open floor holes which measured 33 inches by 33 inches and 8 1/2 inches deep which were not covered, thereby exposing employees to a trip/fall hazard. b. On the first floor, there were eight triangle floor holes which measured 26 1/2 inches by 46 1/2 inches which were not covered thereby exposing employees to a trip/fall hazard.
