29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations: a) In the resin plant at the lime and zinc slurry tanks platform, on or about August 6, 2013 and at times prior to, operators used a fixed ladder to access the platform, resulting in a six-foot fall hazard.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit": a) In the northeast corner of the resin plant packaging area, on or about August 8, 2013 and at times prior to, the emergency egress exit door along the north wall was not identified with an EXIT sign, resulting in a life safety hazard.
