29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about August 21, 2013, employee was exposed to the hazard of falls of approximately 20 feet in height to the ground level below, when working from a Genie S-60 aerial lift and not wearing a safety harness with lanyard, while installing the cornice siding to the building.
