A. All Purpose Contracting, Inc. worksite located at Putnam Parkway, Ottawa, Ohio: On or about August 20, 2013, the employer failed to ensure a safe means of egress such as a ladder was provided to employees installing a storm sewer line in a trench approximately 8.00 feet deep. No ladder was located in the manhole trench box while employees were grading stone preparing to set a precast concrete structure.
