29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that was 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. A. All Purpose Contracting, Inc. worksite located at the intersection of Lakewood Ave. & Cable Rd., Lima, Ohio: On or about July 9, 2014, the employer failed to ensure a safe means of egress such as a ladder was provided to employees installing a storm sewer line and structure in a trench approximately 7.00 feet deep. All Purpose Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 880643, citation number 1, item number 2 and was affirmed as a final order on March 18, 2013 with respect to a workplace located at Defiance, Ohio.

All Purpose Contracting, Inc. worksite located at the intersection of Lakewood Ave. & Cable Rd., Lima, Ohio: On or about July 9, 2014, the employer failed to ensure an employee was protected from cave-in hazards while installing a storm sewer line and structure in a trench approximately 7.00 feet deep. All Purpose Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 880643, citation number 1, item number 3b and was affirmed as a final order on March 18, 2013 with respect to a workplace located at Defiance, Ohio.
