Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    On or about August 22, 2013, the employer failed to provide and ensure the use of seat belts for employees required to operate powered industrial trucks

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):                                                                                                                           a)  On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 100 ton jack, ID# 276211834-3, whose electrical cord outer jack was damaged (where the electrical cord connects to the jack), exposing the cords internal wiring.  b)  On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 100 ton jack, ID# 276211834-3, whose electrical cord had poor stain relief, where the electrical cord connects to the jack.  c)  On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 60 ton jack, ID# ECO 813-3, 69875, whose electrical cord outer jack was damaged (where the electrical cord connects to the jack), exposing the cords internal wiring.  d)  On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 60 ton jack, ID# ECO 813-3, 69875, whose electrical cord had poor stain relief, where the electrical cord connects to the jack.  e)  b)  On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex jack, ID# U43-2, whose electrical cord plug was missing the grounding pin.
