29 CFR 1910.106(e)(2)(ii)(b)(2): The quantity of Category 2, 3 or 4 flammable liquid located outside of an inside storage room or storage cabinet in a building or in any one fire area of a building exceeded 120 gallons:   On or about August 15, 2013:   The employees working in the back production area were exposed to potential fire and smoke hazards.  The employer had more than 300 gallons of flammable liquid stored outside of a flammable storage area or cabinet.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    On or about August 15, 2013:   The employee working in the spray paint area and wearing a 3M half face, tight fitting respirator on a voluntary basis had not received a medical evaluation and the respirator was not properly cleaned, stored, nor was it maintained in a good condition.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceeded the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:      On or about August 15, 2013:     The employee working on the Blue Line in the production area, utilizing the miter saw to cut aluminum and pvc inserts was exposed to an 8 hr time weighted average noise level of 88 decibels.  The employer had not conducted recent monitoring to ensure employees are included in a hearing conservation program.    The Louver Shop was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(d)(1), which was contained in OSHA inspection number 314203563, citation number 1, item number 1 and was affirmed as a final order on December 6, 2010, with respect to the workplace located at 203 Chesterra Drive, Dahlonega, Georgia 30533.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    On or about August 15, 2013:   Employees working in the production area were exposed to chemicals such as, but not limited to acrylic lacquers, lacquer thinner, cleaners, adhesives, and sealers on a daily basis.  The employees had not received initial training prior to working with the chemicals.
