29 CFR 1910.107(b)(10): Illumination. When spraying areas are illuminated through glass panels or other transparent materials, only fixed lighting units should be used as a source of illumination. Panels were not put in place to effectively isolate the spraying area from the area in which the lighting unit is located, and it should be of a noncombustible material of such a nature or so protected that breakage will be unlikely. Panels should be so arranged that normal accumulations of residue on the exposed surface of the panel will not be raised to a dangerous temperature by radiation or conduction from the source of illumination:  a.    On or about August 28, 2013, in the spray booth, the lighting fixtures were not covered with panels used to reduce heat from potential contact of accumulating residue from spraying top coat.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:  a.    On or about August 28, 2013, in the spray booth, an employee was wearing prescription non-safety glasses when spraying top coat onto the wood cabinet pieces.

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:  a.    On or about August 28, 2013, the employer did not provide a respirator fit test for an employee who was required to wear a 3 M half-faced respirator when stain finishing wood cabinet pieces.
