B.) John Anderson Construction, Inc., leased road off of East 5th Avenue, Warren, PA. 16365, on or about August 21, 2013 and times prior thereto: The Kidde dry chemical fire extinguisher (S/N: AP-211552) located on the company pickup truck had not received its annual fire extinguisher maintenance.
