John Anderson Construction, Inc., Leased Road off of East 5th Avenue, Warren Pa, 16365-on or about August 21st, 2013, and times prior thereto: Company employees were required to wear filter facepiece respirators (3M and/or MSA Combo Classic brand half mask respirators with P100 multi-gas cartridges) during cutting operations of old gas line pipe. The Company had not developed and maintained a written Respiratory Protection Program.
