29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest. On or about 22 August 2013, four workers were removing and replacing composition roof shingles on the roof of the building under construction without the use of fall protection, exposing workers to a fall hazard of up to approximately 8 feet and 3 inches to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 22, 2013, four employees were removing and replacing composition roof shingles on the roof of the building under construction without the employer providing fall protection training, exposing the employees to a fall hazard of 8 feet 3 inches to the ground below.
