29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation and unused portion of the running blade: Warehouse, Amada horizontal band saws #3 HA400W, #2 HA250W, and #1 HA250W: On or about August 29, 2013, an employee cutting stainless steel rolls and other metal stock on the three Amada saws was exposed to hand and finger amputation hazards where the unused portions of the running blades were not guarded against the accidental contact. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Mill, Lathe, and Mazak production departments: On or about August 29, 2013, employees operating and setting up CNC lathes and mills were exposed to amputation and caught-in hazards where the safety interlocks were disabled, by-passed, or missing and other protective measures were not taken to prevent unexpected actuation of the machine when working in the danger zone.
