29 CFR 1910.146(d)(5)(ii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing or monitoring the permit space as necessary.       (a) For employees working in the tempering oven permit-required confined space at 1070 Technology Drive, Venice, FL 34275, as noted on or about 09/17/2013. The employees entering and operating in permit confined space did not conduct continuous monitoring of oxygen levels, and sulfur dioxide during permit-required confined space operations.
