COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT 29 CFR 1926.453(b)(2)(v), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314930926 CITATION 01, ITEM 001 AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 11, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 701 SW 19th AVENUE, MOORE, OK.       29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket while working from an aerial lift.     At the worksite:  On or about August 29, 2013, the employer allowed himself and an employee to use an aerial lift without the means of securing themselves to the boom or basket.  The employees were exposed to the type of hazards associated with fall from heights.
