29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about September 17, 2014 and at times prior to employees were exposed to fall hazards working around an unprotected opening on the second floor of commercial building under construction.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about September 17, 2014 an employee was exposed to fall hazards while working from an aerial lift without means to secure themselves to the boom or basket when conducting work activities from the basket at a height greater than 10 feet. The employer, Denver Acoustics & Drywall, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.453 (b)(2)(v) which was contained in OSHA inspection number 936719, citation number 1, item number 1 and was affirmed as a final order on 11-18-2014, with respect to a workplace located at 14120 N. Pennsylvania Ave. Oklahoma City, OK. and OSHA inspection number 314930926, citation number 1, item number 1 and was affirmed as a final order on 04-01-2011, with respect to a workplace located at 701 SW 19th Ave. Moore, OK.
