29 CFR 1910.23(c)(2): Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent): West end of the mud pits: On or about August 29, 2013, and times prior to, employees operating the mud system of a drilling rig during the drilling of an oil/gas well used an unguarded elevated ramp from the mud pits down to the cotton seed hull trailer. This exposed the employees to the hazard of falling over 4 feet.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: East side of the drilling rig: On or about August 29, 2013, employees were operating a drilling rig during the drilling of an oil/gas well where several employees worked on the rig derrick. The ground anchorage for the derrick emergency escape system, Geronimo line, was inadequate. This exposed the employees to the hazard of falling.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Light house: On or about August 29, 2013, employees operating a drilling rig during the drilling of an oil/gas well were provide a dual wheel bench grinder. The grinder left wheel had a 3/4 inch opening between the wheel and the end of the top safety guard exposing the employee to the hazard of being struck by wheel fragements.
