29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: Drilling rig derrick: On or about April 9, 2014, employees operating a drilling rig during the drilling of an oil/gas well worked on the derrick mounted racking platform. The use of the emergency escape (Geronimo) was obstructed by its installation. This exposed the employees to the hazards of fire and explosions.

29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.: Well head: On or about April 9, 2014, and times prior to, employees engaged in the drilling of an oil/gas well worked in the drilling rig substructure around the unguarded well cellar. This exposed the employees to the hazard of falling into the wellhead cellar.
