29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about September 4, 2013, on the 2:12 pitch roof at 6604 Merrill Road, employees were tearing off roofing materials without the use of any fall protection systems, being exposed to a 10-foot 7-inch fall hazard.  b.  On or about September 4, 2013, on the 2:12 pitch roof at 6614 Merrill Road, employees were installing roofing materials without the use of any fall protection systems, being exposed to a 10-foot 7-inch fall hazard.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a. On or about September 4, 2013, to access the 2:12 pitch roof at 6604 Merrill Road, employees used a single section of a 24-foot portable metal extension ladder that extended 11 inches above the landing surface, exposing the employees to a 10-foot 7-inch fall hazard.  b. On or about September 4, 2013, to access the 2:12 pitch roof at 6614 Merrill Road, employees used a single section of a 24-foot portable metal extension ladder that extended 14 inches above the landing surface, exposing the employees to a 10-foot 7-inch fall hazard.
