29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 09/04/13, at residential construction site on Kings Cross Ave.,in the Rock Oak Model Home Center, Clarence, NY: Employees engaged in construction work were not trained to recognize potential hazards on this type of jobsite including but not limited to falls from roofs, ladders, stairs & scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards. ABATEMENT CERTIFICATION REQUIRED

The first cord in the series was plugged into a wall receptacle inside the house under construction. No GFCI or assured equipment grounding program was being used. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 09/04/13, in the garage at residential construction site on Kings Cross Ave.,in the Rock Oak Model Home Center, Clarence, NY: A black extension cord supplied power to a DeWalt Model DWE575 Circular Saw. This extension cord was plugged into one of the three 120 volt-ac receptacles on a DeWalt portable radio/power station. The plug on the DeWalt portable radio/power station lacked a grounding prong, so all three receptacles on the power station were ungrounded. NO ABATEMENT CERTIFICATION REQUIRED
