29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about 9/5/2013 the employer did not ensure the pinch point created between the Line #1 Braner slitter line table and slitter head, over arm take-up reel and steel coil, and un-coiler reel over arm were guarded. b. On or about 9/5/2013 the employer did not ensure the pinch point created between the Line #4 Delta slitter line exit slide table between the slitter rolls and re-coiler, re-coiler reel over arm take up and steel coil, and un-coiler reel over-arm were guarded. c. On or about 9/5/2013 the employer did not ensure the pinch point created between the Line #5 Cincinnati slitter line re-coiler reel over arm and take up reel over arm were guarded. d. On or about 9/5/2013 the employer did not ensure the rotating parts and in-going nip points of the Line #1 un-coiler and take up reel, Line #4 Cincinnati slitter un-coiler reel and take up reel, and Line #4 Delta slitter line un-coiler reel and take up reel were guarded. e. On or about 9/5/2013 the employer did not ensure employees were protected from the steel material moving through the Line #1 Braner slitter line, Line #4 Delta slitter line, and Line #5 Cincinnati slitter line.
