29 CFR 1910.95 (g)(1):  The employer did not establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time weighted average of 85 decibles:     On or about November 12, 2013,  the line 2 operator was exposed to continuous noise levels at 75.2% of the permissible daily exposure (8-hour, time weighted average sound level of 85 dbA or equivalently, a dose of 50%), during the 419 minute sampling period.  Exposure calculations include a zero increment for the 61 minutes not sampled.     On or about November 12, 2013, the line 4 operator was exposed to continuous noise levels at 8505% of the permissible daily exposure (8-hour, time weighted average sound level of 85 dbA or equivalently, a dose of 50%), during the 442 minute sampling period.  Exposure calculations include a zero increment for the 38 minutes not sampled.
