JOB SITE: 191-202 Katy Trail Lane, Saint Charles, MO 63303, at this work site, a worker was observed wearing a protective helmet (hard hat) which the hard hat did not have the protective head band installed inside the hat. The worker was exposed to a struck-by hazard.
