29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. On or about September 5, 2013, at a job site located at 5524 Trammel Estates Drive, North Little Rock, AR 72117, the employer did not ensure employees using pneumatic nail guns were protected by the use of protective eye glasses while installing roof decking. This condition exposed the employees to serious eye injuries.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, personal fall arrest system or an alternative fall protection measure under another provision of paragraph 1926.501 (b). The All Phases Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315715375, citation number 001, item number 004 and was affirmed as a final order on August 24, 2012, with respect to a workplace located at 1109 Anemone Drive, North Little Rock, Arkansas 72117. On or about , September 5, 2013, at a job site located at 5524 Trammel Estates Drive, North Littl Rock, AR, 72117, the employer did not ensure that each employee working and walking on a steep roof, with unprotected sides and edges were protected from falling greater than 10 feet by a fall protection system. Alternative fall protection system use was not attempted.
