29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 8, 2017, at the worksite located at 238 Stonehenge in Cabot, Arkansas, employees traveling below in the vicinity of employees performing framing tasks, did not wear head protection.  Employees were exposed to stuck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 8, 2017, at the worksite located at 238 Stonehenge in Cabot, Arkansas, employees standing on top plates and ceiling joists performing framing tasks did not wear fall protection and were not protected from falling to lower level.  Employees were exposed to falls from elevation of greater than 6 feet.

29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about June 8, 2017, at the worksite located at 238 Stonehenge in Cabot, Arkansas, employees using pneumatic nail guns to perform nail framing tasks did not wear eye protection.  This condition exposed employees to the hazard of eye injuries.  James Carlson dba All Phases Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 940601, citation number 1, item number 1 and was affirmed as a final order on January 21, 2014, with respect to a workplace located at 5524 Trammel Estates Drive in North Little Rock, Arkansas.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 8, 2017, at the worksite located at 238 Stonehenge in Cabot, Arkansas, the portable ladder used to access and descend the top framing did not extend 3 feet above the landing surface.  This condition exposed employees to falls from elevations.   James Carlson dba All Phases Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1178025, citation number 1, item number 2 and was affirmed as a final order on January 31, 2017, with respect to a workplace located at 2845 Plateau Drive in Conway, Arkansas.
