29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:   a)  An open-sided platform on the mixing machine in the ladle repair room did not have standard railings.  The platform only had one chain at the top rail height, creating a fall hazard of greater than four feet to employees accessing the machine.   b)  The Laempe machines in the core room did not have standard railings to guard employees from a fall hazard of over seven feet while accessing the top of the machine to clean the core.
