29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: a) In Shack #6 of the shakeout department, a #6 shakeout operator was exposed to noise at 1142% (noise dose) of the allowable noise dose of 100% during a sample period of 415 minutes during one shift on 11/06/2013. b) In the shakeout department, a shakeout operator was exposed to noise at 1846% (noise dose) of the allowable noise dose of 100% during a sample period of 438 minutes during one shift of 11/06/2013. c) In the shakeout department, a shakeout float employee was exposed to noise at 2199% (noise dose) of the allowable noise dose of 100% during a sample period of 427 minutes during one shift on 11/06/2013. d) In the cooling line of the millroom, a cooling line operator was exposed to noise at 244.3% (noise dose) of the allowable noise dose of 100% during a sample period of 475 minutes during one shift on 01/24/2014. Applicable administrative or engineering controls may include, but are not limited to the following: 1) Conduct thorough noise studies in the buzzer shacks and cooling line to verify noise sources, source noise frequencies, reflective surfaces and employee operations/positions for most effective engineering controls. 2) In the cooling line, add damping material to the outer surfaces of the transfer collars. 3) In the buzzer shacks, add damping materials to hard reflective surfaces to reduce reflection of noise into the employees' hearing zones; ensure high frequency buzzers are vibration isolated from buzzer shacks; increase the distance or isolate buzzer employees from the high frequency buzzers. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): a) On 11/13/2013, a Line #5 grinder employee in the millroom, was overexposed to respirable dust containing 15% crystalline silica (quartz) at a level of 1.2 mg/m3 as an 8-hour TWA which is approximately 2 times the calculated 8-hour time weighted average limit of 0.588 mg/m3. General methods of compliance include, but are not limited, to the following: 1) Conduct a thorough study of air flow patterns throughout Line #5 grinding stations to verify that air flow is not bringing contaminated air into the employees' breathing zones. 2) Check the ventilation systems for the Line #5 grinders to verify that the exhaust systems and clean air supply systems are working within their specifications. Make any adjustments or repairs as needed. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
