29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: Jobsite: Shop Two (2) pieces of cab glass were broken on the Combilift CB. The windshield was shattered into a spiderweb pattern, adversely affecting visibility and the safe operation of the vehicle, and the rear glass was also broken.

29 CFR 1910.180(d)(1): Prior to initial use all new and altered cranes were not inspected to insure compliance with provisions of this section: Jobsite: Shop. A 1963, 65 ton, American model 975, which had been newly assembled, returned to service, and used, had not been documented as having been inspected by a qualified person.
