29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: Front Street Job Site - On or about 9/5/13, excavation operations did not exactly locate the underground gas line by a safe means as the excavator hit an the gas line rupturing it.
