OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of explosion and fire. a) On or about September 4, 2013, in the Raw Sugar Bin Area, automatic sprinkler heads covered with plastic sandwich bags were covered with sugar residue interfering with intended operation. b) On or about September 4, 2013, in the G-belt tunnel, automatic sprinkler heads covered with plastic sandwich bags were covered with sugar residue interfering with intended operation.

On or about September 04, 2013, in the Storeroom area, the employer allowed employees to use an extension cord as a power source for a Daytona fan for a four month period. The extension cord was attached to the fan for a year.
