29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. At the jobsite located at 6000 SE 2nd St., Tecumseh KS (Innovia Films): Employees engaged in structural constuction activities in the basement below line 5 were exposed to the release of extremely flammable vapors to the atmosphere that historically result in fire / explosion hazards in that fire retardant clothing was not provided while working below the covered process.
