29 CFR 1910.137(b)(2)(ii): Insulating equipment were not inspected for damage before each day's use and immediately following any incident that can reasonably be suspected of having caused damage. Insulating gloves shall be given an air test, along with the inspection. On or about July 16, 2013 employees were wearing insulated electrical gloves that had not been inspected for damage prior to each use.

Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6. One or more methods of machine guarding wer not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) on or about August 12, 2013, employees were exposed to pinch point hazards on the automated powerhead machine due to lack of effective machine guarding." b) On or about October 16, 2013, employees were exposed to pinch point hazards on the varnisher machine due to the lack of effective machine guarding."
