29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that would have prevented employee exposure to hazards: a) Rt. 202, Worksite, Excavation - on or about September 9, 2013, employees were installing a new sewer line within an excavation while working within a GME trench shield that was not maintained in a manner consistent with manufacturer's instructions, in that, only three out of the four required spreaders were installed.
