29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     A)    Tooling Technology, LLC, located at 4711 North State Route 19, Fremont, OH 43420: On or about September 12, 2013, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of the CNC machine # 6 (Viper 2100, Model HV-80S, Serial # 001186).  Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic, while performing activities such as, but not limited to, changing set-up of the machine. Employees were exposed to amputation hazards.       B)     Tooling Technology, LLC, located at 4711 North State Route 19, Fremont, OH 43420: On or about September 12, 2013, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of the CNC machine # 15 (HAAS, Model VF-10, Serial # 17751).  Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic, while performing activities such as, but not limited to, changing set-up of the machine. Employees were exposed to amputation hazards.       C)     Tooling Technology, LLC, located at 4711 North State Route 19, Fremont, OH 43420: On or about September 12, 2013, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of the CNC machine # 10 (HAAS, Model VF-3, Serial # 5281). In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Employees were exposed to injury from unexpected start-up and energy release. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. Employees were exposed to struck-by and amputation hazards.       B)     Tooling Technology, LLC, located at 4711 North State Route 19, Fremont, OH 43420: On or about August 12, 2013, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, rotating chuck of the CNC machine # 15 (HAAS, Model VF-10, Serial # 17751). Employees were exposed to struck-by and amputation hazards.
