29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers was not provided, used, and/or maintained. a) Tooling Technology, LLC - Fort Loramie, Ohio: On or about April 11, 2014, employees who operate hand-held pneumatic die grinders equipped with a crimped wire end brush to grind castings are exposed to struck-by hazards. The employer did not ensure that employees used personal protective equipment for the arms and/or face to protect against flying debris from the wire brush. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
