OSH ACT of 1970 Section (5)(a)(1):       The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushing injuries:       Employees working throughout the facility were exposed to the hazard of being crushed by loads suspended from under the hook lifting devices that were not marked with their rated load, or identification, and had not been inspected on a periodic basis.         a.     The four 192 inch 2 Point Wagon Lifts were not marked with their rated loads or serial numbers and were not inspected on a periodic basis in accordance with the standard.        b.     The four 164 inch 2 Point Wagon Lifts were not marked with their rated loads or serial numbers and were not inspected on a periodic basis in accordance with the standard.     c.     The eight 136 inch to 180 inch 1 Point Wagon Lifts were not marked with their rated loads or serial numbers and were not inspected on a periodic basis in accordance with the standard.        d.     The Sheetlifter was not marked with its rated load or serial number and was not inspected on a periodic basis in accordance with the standard.       e.     The 6 Spreader Bar was not marked with its rated load or serial number and was not inspected on a periodic basis in accordance with the standard.        f.      The 3 Spreader Bar was not marked with its rated load or serial number and was not inspected on a periodic basis in accordance with the standard.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    Next to the roller machine, the employer did not ensure that a flexible extension cord attached to the 220 volt gang box had proper strain relief. The flexible extension cord had its outer insulation pulled away exposing employees to electrical shock hazards when in use.
