OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushing or falling when trailers were moved away from the loading dock while dock workers were engaging in loading or unloading material:   Loading docks:  The Rite-Hite Dock Light Communication System used for communication between the drivers and the loaders was not maintained in proper working order in the following instances:  a)  Door 4 - the outside lights had been switched so that the green light was lit outside at the same time that the green was lit inside, and the red light was lit outside when the red light was lit inside; b)  Door 5 - the outside light did not switch when the inside light was switched; c)  Door 18 - the outside green light cover was broken; d)  Door 20 - the outside green bulb was not working; e)  Door 36 - the outside red bulb was not working; f)   Door 37 - the outside red and green bulbs were not working; g)  Door 38 - the outside green bulb was not working; h)  Door 39 - the outside green bulb was not working.
