29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    Establishment:    Lockout tagout procedures for maintenance and repair on the corrugated cardboard & plastic wrap baling machines did not clearly and specifically outline:  a) Intended use of the procedure.  b) Specific procedural steps for shutting down, isolating and blocking machines or equipment to control all hazardous energy.  c) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    Freezer:    Materials were not stacked, stable and secured against sliding and/or collapse creating overhead hazards:  a) Aisle 81.131 G 01:  A pallet of frozen food located on a storage rack approximately 32 feet above ground level was damaged.  b) Aisle 80.131: Boxes of frozen food product on a pallet located on a storage rack more than eight (8) feet above ground level were not stacked in a secure manner.
